"Failure to obey a subpoena without being excused is a contempt of court." The form provides either for a signature of the court administrator or an attorney acting as an officer of the court. Includes return of service form, a summary of Minnesota Rule of Civil Procedure 45 and information about witness fees according to Minn. Stat. § 357.22 and § 357.242.
